Research Study -- Become a part of an important research study regarding the impact that our life experiences have on each of us and our descendants. 

Life Recollections Family History Services invites you to become part of a survey which is taking place to document a cross-section of America as it relates to various questions concerning some Family History topics.  Your participation and honesty become an important component to the successful completion of this survey.  

Since some of your answers might be used in media or various publications, we might ask that you also complete a Media Release form.  So, it is important that we be able to contact you in the event that we would like to include your photograph to accompany any quotes you have made.  If you wish to remain anonymous, your participation is still appreciated and your name will never be used.

Questions:

1)   Who was the most influential person in your life and why?
2)   What important Life Lessons have you learned – these can be positive or negative lessons learned.  
3) Is there a favorite quote or saying that has been passed along to you from a parent or individual who was influential in your life?
4)   What is the hardest choice that you have had to make and why was it difficult for you?
5)   What legacy do you want to pass along to future generations in your family about you or your accomplishments and/or struggles (you can give detailed reasons for your answer)
6)  Please indicate what age group applies to you:
